You can fill in our New Patient Registration Form online and submit it with a click of your mouse button.  This will save time when you come for your initial appointment.

If you are coming to the office for naturopathic care, we ask you to fill out a more extensive health history form.  These forms are available below.

• Fax us your printed and completed form(s) or bring it with you to your appointment.
